Terms and Conditions for Use of the Florida Hospital Healthy 100 Member Discount Card
- The Membership Discount Card is valid only at participating vendors and locations, as indicated on the Healthy100.org website.
- Offers are only valid as stated on the Healthy100.org website.
- The Discount Card must be presented at time of purchase to receive the offer.
- Discount Card offers may not be valid in conjunction with other offers or discounts.
- Specific restrictions may a pply as determined by the vendor.
- Vendor discount offers are subject to change without notice.
- Florida Hospital has the right to modify, add, delete or discontinue all or any portion of the discount program at any time without notice.
- The Membership Discount Card has no cash value, and Florida Hospital has no obligation to pay or reimburse members for any amounts in connection with the Discount Card.
- Membership Discount Cards are not transferable.
- Florida Hospital will mail one Membership Discount Card to each Healthy100 Member who provides a physical address within the Florida counties of: Orange, Osceola, Seminole and Lake. Florida Hospital is not responsible for lost, misplaced or stolen cards.
- Florida Hospital will provide only one replacement card, which must be picked up at the location indicated by Florida Hospital and will not be mailed.
